1. Once the QR code is scanned, your patient will land on a page that looks like this. For our larger medical group partners we will customize this page with your branding, as well as the Care Directives shown below.

2. The QR code is unique to your practice, and creates a feedback loop in which you will be kept up-to-date on patient progress. 

3. Once your patient clicks on directive below that matches their referral card, they will complete a quick and easy questionnaire, which initiates the start of care and/or monitoring.
